What kind of personal information do we collect about you?

In order to provide a service to you, we collect and hold the following kind of information about you:

·       Name, address, date of birth, email address, postal address, telephone number and other information relevant to your reason for seeking physiotherapy.

·       Information about and any communication with other health professionals involved in your care

·       Other relevant health information about you such as a record of your symptoms or health concern, your relevant medical, family and social history, any diagnosis made and the treatment we give you.

·       Also

-          specialist reports

-          test results

-          your appointment and billing details

-          other information about you collected for the purposes of providing care to you.

 In order to effectively carry out a treatment plan, each contact you have with SCP will be securely recorded.

How do we collect your personal information?

We will generally collect personal information about you in these ways:

  • Directly from you when you give us your details (eg, face-to-face, over the phone, email, text message, via a booking form or an online form, a paper form or information otherwise furnished by you)

  • From a person responsible for you or who expresses concern about you

  • From a third party where we are permitted by you, or by law to do so (eg. other health care professionals involved in your care, from your health insurer)

How do we hold your personal information?

We take reasonable steps to protect the information we hold about you. These are designed to
prevent unauthorised access, modification or disclosure and to prevent misuse and loss.
This includes:

  •     We will retain and store all information provided securely, in a locked cabinet in a lockable room or GDPR compliant third party, encrypted, cloud-based software.

  •   In some circumstances it may be necessary for these third-party providers to access your information in the course of communication, booking, payments and provision of electronic treatment information. Sinead Curran Physiotherapy has ensured that, although these companies may not be located in the EU, they declare GDPR compliance. They are well established and well used in the physiotherapy community in Ireland. Sinead Curran Physiotherapy is not responsible for the practices of these websites. Each has a copy of their own privacy policies available to view.

www.stripe.com  (collection and processing of fees

  •     We will only keep records for as long as we are required to by law.

  •   Your information will never be sold to a third party.

 

Why do we collect and use information about you?

We primarily collect and use personal information about you to provide our physiotherapy services to you and to communicate with you and others involved in your care in relation to those services.

The contact information you provide is used to communicate with you. Unless otherwise requested by you, this communication is relevant to your enquiry (in the case of an initial enquiry contact) or to your episode of care only.

You have the right to object to, or restrict, the processing of your personal data for a particular purpose.

We occasionally use information for other purposes, including:

  •    To help us manage our accounts and administrative services, including billing/ accounts management and management of our IT systems

  • To conduct accreditation, quality assurance or internal audits.

When and why might we share information about you with others?

We may disclose information about you to others outside of our practice as permitted or required
under law. This will include situations where we disclose information about you in order to:

  • Comply with our legal and statutory  obligations (eg. mandatory reporting under legislation,
    responding to a court order or subpoena)

  •   Lessen or prevent a serious threat to a patient’s life, health or safety or a serious threat to public health or safety

We may also share information, with your express permission, in order to:

  • Claim on insurance, if relevant

  •   Get test results from diagnostic and pathology services from a third party

  • Communicate with your doctor or other health care provider involved in your care.

 Only necessary information is communicated, secure electronic communication is used and,

where possible, is via www.healthmail.com

  •   In some situations, Healthmail is not an option. In these cases, with your consent,

       telephone conversation, Google Workspace (2FA) or An Post surface mail is used.

 

Your Rights

  • You have the right to seek access to and to seek correction of the information we hold about you

  • You have the right to seek access to and correction of the data we hold on you.

  • We will provide a copy of the record to you or a designated third-party (e.g. solicitor) upon receipt of written request and appropriate consent.

  • This request will be completed within 30 days and there is an administration fee of €20.

  • If you think that the information we hold about you is not correct, let us know in writing. We will
    take reasonable steps to correct your personal information where the information is not accurate,
    or up-to-date. From time to time, we may also ask you to verify that the information we hold about
    you is correct and current.
